Supportive Services
At Providence Family Life Center, we provide health education, economic services, and housing opportunities, via principal points of contact made by an integrated health services team of board-certified practitioners, social workers, physicians assistants, and mental health professions leveraging a unique model of services that “meet people where they are,” with the aim of alleviating the negative impact of the Covid-19 pandemic and hurricane IAN.
Social Security Disability
The SSI/SSDI program helps individuals with access and functional needs age 55 and older who are living with a disability and are unable to work. If you meet the eligibility criteria, we can help you apply for SSA disability benefits.
Representative Payee
We help beneficiaries 55 & older manage monthly SSI benefits, understand how to create a budget, make bill payments, and establish a bank account. We also help with explaining documentation and filing the appropriate information for SSA continuing disability reviews.
Chronic Disease & Self Management Workshops
Chronic Disease Self-Management Programs (CDSMP), have enhanced the lives of hundreds of thousands of people around the world. Our small group 6- week workshops are for people with chronic disease and their caregivers. Participants learn how to manage medications, the benefits of good nutrition and exercise in a supportive environment.
We work with individuals, families and health providers to manage care. Our Life Care Plan can help you make important decisions related to health care, finances, personal property and real estate, and other situations. We can assist with long-term care planning such as independent and assisted living, memory care and advanced directives.
